NEET Self Declaration Form 2024 – The NEET undertaking is a self declaration form issued by the National testing agency along with the NEET 2024 admit card. It is done by the exam conducting body to ensure that the candidates appearing for the exam are not having any COVID 19 symptoms. How to Fill NEET Self-Declaration Form 2024 & NEET Undertaking?

The NEET 2024 self declaration form consists of four sections. The candidates are advised to fill all the sections with utmost care because it will be checked at the NEET exam centre. The NTA admit card undertaking details are :-

  •  Declaration by the candidate.
  •  COVID 19 related details of the candidate.
  •  Entry to the exam center.
  •  Photograph, signature and thumb impression of the candidate.

NEET Self Declaration 2024 Form Filling:-

The first part of the admit card undertaking is the self declaration by the candidate. Through this, the NTA asks the candidates to self declare that they are aware of the government guidelines regarding the COVID 19. 

It further asks the candidates to confirm that they have read the information bulletin, notices and instructions related to NEET UG 2024 on the official website (neet.nta.nic.in).

NEET 2024 admit card undertaking – COVID 19 related details of the candidate :-

In the second section of the undertaking, the candidates will have to declare if they have had any flu like symptoms in the past fourteen days. They will also have to confirm whether they have been in close affinity to any person diagnosed with COVID 19 or having those symptoms. They will have to mention their travel details of the past fourteen days. They will also have to mention the names of cities/ countries that they have crossed to reach the exam center city.

NEET 2024 admit card undertaking – Entry to the exam center:-

In the third section of the admit card, the National testing agency takes the responsibility of the health and welll being of candidates. Thus it states to have the right to deny entry to any infected person, only to avoid the risk of infection.

NEET 2024 admit card undertaking- Photograph, signature and Thumb impression :-

In the fourth section of the undertaking, the candidate will have to declare that they have understood everything mentioned in the NEET UG 2024 admit card undertaking. Candidates will have to paste a passport size NEET photograph (as on the admit card), put their left hand thumb impression and do their signature ( only in the presence of the invigilator). The required space for photograph, signature and thumb impression will be provided by the NTA on the admit card undertaking itself.

How to download the NEET Self Declaration Form 2024?

For downloading the NEET Self Declaration Form 2024, the candidates will have to follow the below mentioned steps:-

  • Visit the official website of NTA ( neet.nta.nic.in)
  • Click to download the admit card
  • Login using NEET application number and date of birth.
  • The NEET Self Declaration Form 2024 will be downloaded along with the admit card.
  • Read the undertaking carefully.
  • Take multiple printouts for further reference.

What to carry along with the NEET Self Declaration Form 2024?

Candidates are strictly asked to carry the following documents to their respective exam centers:-

  •  NEET UG 2024 admit card.
  •  NEET UG 2022 admit card undertaking
  •  Valid ID proof
  •  Passport size photograph
